Enrolling: Guardianship



A student residing with someone other than parents or legal guardians within the boundaries of the Lake Oswego School District, and who wishes to attend LOSD schools, must submit the following district guardianship papers. Forms should be submitted together:

Parent Assignment of Responsibility (pdf)

Guardian Assuming Responsibility (pdf)

Note:  You must have Adobe Acrobat Reader to access these documents.

If the student has legally appointed guardians, district forms need not be completed, but proof of legal guardianship must be presented.

Completed forms should be forwarded to the Office of Educational Programs at the  District Administration Building

Decision process generally takes 2-3 business days.

Upon approval or denial, a copy of the request will be forwarded to the school that the student will be attending, to the parents, and to the guardians.
